PDF Printable Registration Form

Oh boy. This is the page where the printable patient registration information should be. We have one of two options here. We could either create a link to a pdf or word doc, or we could create a form online which the patient could submit electronically.

The first option is nice because the patient could come to their appointment with their paperwork in hand and ready for review. It might require some disclaimer saying that the clinic does not claim legal responsibility for any lost medical history charts. If the patient is printing something out, we probably need to make them enter into an agreement which says that, once they hit that print icon, they are responsible for protecting their own medical history and whatnot.

The second option will take a little bit of time, making a form on the web would probably take a couple hours as opposed to a few seconds of cutting and pasting code, but it would be pretty flash. The content of the "Preparing for your Visit" page should hinge on the way the forms are set up.
